Feeling pressured to sign N11 form by landlord

My landlord is insisting I sign this N11 form, which seems like an agreement to end my tenancy. I'm confused about whether the rent includes tax and if there are hidden fees. Do I really have to sign this? It feels like they're trying to kick me out. What's my rights as a tenant?
