Partner moving out unexpectedly - can I take over the lease?

My partner just got a job offer in another city and will move out in 3 weeks. We have 6 months left on our lease. I'm worried about affording the rent alone on my entry-level salary. Has anyone navigated taking over a lease by themselves? What steps should I take?
