Need to break my lease early - what are the penalties?

I'm in a tough spot and really need to break my lease early due to a job change. I'm stressed about the potential penalties and really just want to know what my options are. Can I assign my lease to someone else or sublet? I'm worried about how to get out of this lease without huge costs.
