Landlord wants me to sign N11 form - do I have to?

I’m feeling really pressured by my landlord to sign this N11 form. They keep pushing for me to agree to move out, and I'm suspicious about their motives. Do I have to sign this agreement? I don't want to be kicked out without knowing my rights as a tenant.
