Need advice on breaking my lease early - stressed and seeking options

I'm feeling really stuck in my lease and need to leave early due to personal reasons. I've heard about N11 agreements and subletting, but I'm unsure how to approach my landlord or avoid penalties. Any advice on getting out without too much hassle would be greatly appreciated!
