Frustrated tenant needing repair rights advice ASAP!

I'm at my wit's end! My landlord refuses to fix major issues like a leaking ceiling and broken heater. My lease is ending soon, and I'm running out of options. I've heard about T6 applications and rent abatement, but I'm not sure how to proceed. What can I do to force repairs?
