Frustrated tenant needing help with landlord repairs

I've been living in my Toronto apartment with leaks, mold, and broken appliances for months. My landlord keeps brushing me off and refuses to make any repairs. I'm at my wit's end! Are there legal options, like T6 applications, for forcing him to fix these issues? I can’t live like this anymore!
