Stressed out tenant needs help with repairs and landlord troubles!

My landlord has ignored multiple requests for urgent repairs—leaking pipes and mold spreading everywhere! I'm at my wit's end, and now facing eviction. I need to know my rights for T6 applications, rent abatement, and how to force them to act. Any advice?
