Stuck in Lease - Need Help Breaking It Early

I'm really stressed about my current situation and need to leave my lease early in Toronto. I'm uncertain about the penalties for breaking the lease and whether subletting could be a viable option. Also, how can I approach my landlord for an N11 agreement? Any advice would be appreciated!
