Partner moving out - can I take over the whole lease?

I'm in a lease with my partner who is moving out at the end of January. We signed for one year, but I want to stay in the apartment and take over the lease. I’ve talked to my landlord, who said they might agree, but I need to know the process. Has anyone been through this?
