Partner moving out, can I take over the whole lease?

My partner is moving out at the end of the month, but we're on a joint lease until December. I'm planning to stay in the apartment, but I'm not sure how to go about taking over the lease entirely or if I need my landlord's permission. Has anyone navigated this before? What should I do?
