Stuck in a lease - Need help breaking it early legally

I'm feeling really stressed out about my lease situation here in Toronto. I need to leave early but I'm worried about penalties for breaking the lease. What are my options for subletting? How can I approach my landlord for cooperation? Is there a way to do this without getting stuck in a legal mess?
